Charnwood Oaks Nursing Home Care Quality Commission

They added that, while residents said they ‘had never felt unsafe’ in the care home, there were not enough staff to ensure that all areas of the home were monitored properly. This meant there was the possibility for falls and injuries to go unnoticed. Inspectors reported one resident had visible bruising, but staff could not explain where it had come from. Other resident injuries were similarly not investigated properly, leading the inspectors to say they had concerns that patients were not properly protected from possible abuse. The CQC had received a number of concerns about the service before visiting. The Scottish Care Inspectorate is the official regulator for care services in Scotland.

Relatives told us that people were treated with dignity and respect. One relative's comments were typical of what others told us. They said, "The staff are very caring. They treat people with dignity and respect." People's health and care needs were assessed with them or their relatives. Care plans included details of people's needs and information about how they were supported with their needs.

Where are they located?

The provider had a system for monitoring the quality of service. This included checks of documentation, records and observations of care worker's practice. We saw from records of those meetings that the manager had shared information about the outcome of monitoring activity. One person told us they had experienced, "tender love and care" and added, "Even when staff are rushed they don't let you think something is too much trouble." People told us that staff were "kind".

We saw people receiving care and support and observed the staff respecting people's dignity at all times. The service had procedures for reporting of accidents and injuries. We saw that reports were reviewed and analysed and that action had been taken to reduce the risk of the same type of accident occurring again. The registered manager regularly sought the views of people who used the service and their relatives.

People’s mental capacity to consent to their care had been assessed where there was a reasonable belief that they may not be able to make a specific decision. Live-in care is when a fully trained carer lives in the home of the person needing care, allowing them to remain in their own home, maintain independence, and get the dedicated care support required. – there are some services which we can’t rate, while some might be under appeal from the provider. Suspended ratings are being reviewed by us and will be published soon.

We saw lapses by a very small number of staff which we brought to the provider’s attention and they told us action would be taken to address this through training and closer supervision. The service had not always responded promptly when people had experienced unplanned weight loss, for example by involving dieticians in people’s care. A safeguarding investigation by the local authority found that a person’s health had not been adequately monitored and that this was a contributing factor to a serious incident that occurred.

Some people told us they would like more opportunities to take part in activities in the home. Not all staff were aware of how to report concerns about people’s safety and how to report and escalate issues and incidents if required. The building was in need of some maintenance and improvement. Redecoration was required and flooring in some areas needed to be replaced. – the service is performing well and meeting our expectations. Our infection prevention and control inspections look at how well people using a service are kept safe from the spread of infections. Staff told us that people who were at risk of malnutrition had food and fluid charts completed for them on a daily basis. We reviewed some of these charts and found there were gaps in the recording of people’s food and fluid intake.

Caring

Staff interactions with people while generally positive were task focused and people did not feel that they were listened to. People were not supported to engage in activities or follow their interests. People were not always supported to make decisions about the care they received. The provider had considered their responsibility to meet the requirements of the Mental Capacity Act and Deprivation of Liberty Safeguards . The registered manager was aware of their responsibility to report events that occurred within the service to CQC and external agencies. People were supported in line with the requirements of the Mental Capacity Act.

My father lived and was loved at Charnwood Oaks for almost two years, following a lengthy stay in hospital which rendered him disabled and unable to care for himself at home. Staff quickly reassured him and made him feel safe, and established a warm, fun and caring rapport. He loved their company and I would say that he was happier there for those two years than he had been for a long time previously. He enjoyed trips out, even though these were difficult to manage, his being wheelchair bound. The family get togethers, such as the Christmas party and the annual bonfire night and garden party were wonderful occasions when families joined in.

The Care Quality Commission is the official regulator for the care sector. They inspect care homes and care services to make sure they are safe, caring, effective, responsive and well-led. Ash Tree is a small care residence located in Loughborough, with a capacity of six. During our inspection visit we spoke with people using the service and with relatives and friends who were visiting. All were satisfied with the care and support provided at Charnwood Hall.

The provider also used a satisfaction survey to obtain people's views. We saw that staff treated people with dignity and respect. Staff spoke politely to people and offered encouragement when they supported them. Staff we spoke with, including staff with no care work responsibilities, all knew the forms of abuse recognised in the Health and Social Care Act 2010.
